4. Assembly, Maintenance, Inspection, and Disassembly Procedures

Assembly and disassembly of all equipment will be done according to the manufacturer’s recommended procedures. (Include copies of the manufacturer’s data for each specific type of equipment used.)  Designate who will be responsible for each task and what level of training they have.

Types of equipment, machinery or tools on the worksite are:

A comprehensive, careful, and visual inspection of all safety equipment will be conducted daily or before each shift or its use, as highlighted and described in the Employee Training Packet (ETP).  Any defective equipment will be tagged and removed from use immediately. The manufacturer’s recommendations for maintenance and inspection will be followed. 

5. Handling, Storage & Securing of Tools, and Material

Toe boards will be installed on all scaffolding to prevent tools and equipment from falling from the scaffolding. Another specific handling, storage and securing are as follows:  

6. Overhead Protection

Hard hats are required on all job sites except for those that have no exposure to overhead hazards. Warning signs will be posted to caution against existing hazards whenever they are present.  In some conditions/cases, debris nets may be used if a condition warrants extra protection.

Additional overhead protection will include:

Toe boards (at a minimum of 4 inches in height) could be installed alongside the edge of scaffolding and walking surfaces for a distance enough to shield personnel below. Where tools, systems or materials are piled better than the top of the toe board, panelling or screening can be erected to protect personnel below.

7. Injured Worker Removal

Normal first aid procedures should be carried out as the situation develops.  If the area is safe for entry, the first aid should be done by a foreman or other certified individual. Initiate Emergency Services – Dial xxxxx (where available)

Proper Rescue considerations. When personal Fall Arrest Systems (FAS) are used, the employer must make sure that the employees can be promptly rescued or can rescue themselves should a fall incident happen. The quick, ready availability of rescue personnel, ladders, or other rescue equipment should be evaluated carefully and very closely.  In some circumstances, equipment and or tools that enable employees to rescue themselves after the fall has been arrested may be desirable, such as devices, equipment and or tools that have sufficient capability.

8. Training and Instruction Program

All newly hired workforce/employees will be provided and inducted with instructions on the proper and safe use of Fall Protection Devices (FPD) before they begin work.  The employee/workforce will sign a form describing that they have been given this proper and essential information. This form becomes part of the employee’s personnel file history.

The written Fall Protection Plan (FPP) will be reviewed before work begins on the job site. Those employees attending will sign below. The fall protection equipment used will be reviewed regularly at the weekly safety meetings.

Before permitting employees into areas where fall hazards exist, all employees must be trained regarding fall protection plan requirements. Proper and careful Inspection of fall protection devices/systems must be made to make sure compliance with Occupational Safety and Health Administration (OSHA) and (Company Name) internal regulations.
